Health Assessment for Patients 45 to 49 Years Old
The Federal Government introduced a Health Check for all patients aged between 45-49-Years-Old who may be at risk of developing a health condition such as diabetes, heart disease, or other long-term chronic diseases. The aim of the health check is to help identify, prevent, or lessen the effect of the disease.
This health check will give us the opportunity to look at your lifestyle, medical, and family history to determine if you may be at risk. The nurse conducts the assessment, and you are then seen by your GP to discuss the findings.
If you think you are eligible and would like to book in for a 45-49 Health Assessment, please contact reception. This appointment must be in the clinic and cannot be a telehealth.
